Dogs and cats from ALL towns are welcome. (Please bring your cat in a carrier. Dogs must be leashed.)

Complete one form for each pet.
Be sure to provide complete details for rabies vaccinations and microchips.
Return the form(s) to PAWS by email to pawswakefield@outlook.com or by fax to 877.866.1406.
IMPORTANT: Be sure to send in your registration form(s) byThursday, April 27, to qualify.
PAWS volunteers will get back to you by phone, text or email to let you know your priority number and approximate time slot for the clinic.
Payments may be made by cash, check, or charge, onsite at the clinic. |
THREE-YEAR RABIES VACCINATIONS
If you want your pet to have a 3-year rabies vaccination,
you must bring proof of your pet's last vaccination.
